Pepsi Youth Bowling Championships

Please be sure to follow the instructions, this form will register you for the Pepsi State Finals only, If you qualified for the Handicap Scholarship Singles, please go to the registration page for that event. Be sure to complete all information requested.

Form Instructions:

Please complete all fields, do not leave anything blank, all fields are required.

Click the "NEXT STEP" button when finished. You should then see a page to verify your responses. If satisfied, then press the "SUBMIT" button at the bottom of the page. This will then submit your data, your data is not submitted until you click on the "SUBMIT" button and you see a THANK YOU page.

CAN NOT ATTEND: If you are a qualifier and cannot participate, please complete the form and select the appropriate field in the "squad choice" so that we may notify the next available alternate.

You will also get a confirmation email sent to the email address entered on the form, if you missed typed the email address, you will not receive such an email. Please check the State Confirmed Finalists pages to see if your first choice was offered. We will contact you if that is not the case, provided we have a valid email address to work from.
